Connected-BP is a program for patients to check their blood pressure readings at home over a two-week period and transmit them for their clinicians' teams to review in PennChart.
After a clinician places the Consult to Connected-BP order, we recommend dropping the smartphrase, known as .connectedbp, into the patient's After Visit Summary. This will populate a QR code, linking a Connected-BP Patient Education Video for the patient to watch on Youtube. For more information on this program, Click here.
We recommend providers enroll their patients who possibly have white-coat or masked hypertension. However, PCDC would suggest excluding patients who might require response within 24 hours of texting in their BP readings. Ultimately, patient enrollment is at the discretion of the clinician.
You should receive a call from our team within 24 to 48 hours of the order placement by the patient's clinician. At the time of enrollment, we will provide clear instructions on how to check and text your BP readings via your cellphone as when to check your BP during the day.
You will need a personal cellphone to have the ability to text your BP readings into your phone, and we will share those readings with your clinician's team. If you do not have a BP cuff, you should plan to buy one at your local pharmacy or retail store for $45 - $70.
